2024 NAACOS Excellence Awards

Overview

The NAACOS Excellence Awards recognize high performing organizations who have demonstrated an outstanding commitment to and accomplishments in value-based care. Recipients of the award exemplify how value-based care can transform health care delivery by improving quality and outcomes, lowering or slowing growth of costs, and increasing patient and provider satisfaction.

The call for submissions will be open from July 1 – Aug 2, 2024. Awardees will be notified in early September. Awards will be presented at the NAACOS fall conference October 16-18 at the Marriot Marquis in Washington, D.C.Awardees will select a representative to accept the award and present as part of a panel discussion outlining their submission.

Award and Review Criteria

The award/review criteria were developed to gather information to support the selection of organizations successful in value-based care and to reflect the diversity of ACOs and value-based care entities. Each of the five categories are equally weighted.

  1. Patient and Community Engagement – In what ways does the organization engage patients and the community it serves in its value-based care efforts. (20% of overall score)
  2. Value Culture – How does the organization demonstrate a culture of commitment to value-based care. (20% of overall score)
  3. Value Proposition/Focus on Outcomes – How does the organization demonstrate success in value-based care. Please include objective outcomes that support your submission (quality scores, savings generated, results of a particular innovation or intervention, and others you feel support your submission). (20% of overall score)
  4. Use of Data and Tools – How are data and tools used to improve patient outcomes and reduce costs. If applicable, please include a description of how you use this information across populations. (20% of overall score)
  5. Clinical Interventions – What specific clinical interventions are used to improve patient outcomes. If applicable, please include a description of how you use these interventions across populations. (20% of overall score)
