This session will demonstrate how two healthcare systems use their teams for care navigation across networks to improve TOC measures and patient outcomes.  These include using Care Navigators + a manager for TOC management across 8 networks utilizing realtime ADT notifications for CN team in Pop Health platform for ED, inpatient, SNF discharges with 2-day f/u, SDOH assessment and ongoing program enrollment. This will be followed by working examples of expanded integrated services provided by Community Health Workers, RPM, Med rec, Paramedics, Pharmacists, SNF waivers, TOC ACO Provider, Communications, and SNFist
