Back to Baseline: Harnessing Advanced Analytics to Improve Post-acute Outcomes

When ACO patients leave the hospital, some require more attention than others. But there is no time to conduct a deep-dive into the patients’ full medical history or analyze their current risk of readmission and adverse events. So, the default is to either reach out to all discharged patients, go through a list in the order that the names came in, or hope that their PCP will address post-acute care needs. Critical Policy Updates for ACOs

In this session, NAACOS staff will review key policy issues affecting ACOs and Direct Contracting Entities (DCEs), including updates on the public health emergency, key policies included in the proposed 2023 Medicare Physician Fee Schedule, and CMMI model updates. It will also include an overview of NAACOS’ Congressional advocacy to extend the 5 percent Advanced APM bonus and grow participation in ACOs. The session will also feature a Congressional staff panel so attendees can hear directly from the staffers at the heart of the lawmaking process on Capitol Hill.

Community Engagement Strategies to Address Social Determinants of Health and Reduce Unnecessary Admissions and Readmissions

ACOs are developing strategies to address social determinants of health and improve health equity – two major priorities of the Biden Administration. This session will examine how one ACOs is building a comprehensive strategy to address SDOH including evaluating patient’s social risk factors, building a network of community-based social service organizations, and developing an effective closed-loop referral system. Two major challenges facing ACOs is the lack of sufficient community-based capacity to deliver social services and need to develop sustainable partnerships with community-based organizations (CBOs). The session will discuss the characteristics of successful partnerships provider from the perspective of one well-established CBO network. It will also discuss federal efforts to strengthen community social service capacity including the development of one-stop community resource hubs.

In this session Task Force Chair, Dr. Katherine Schneider, will discuss key issues for CMS, the vendor community and other stakeholders to consider as ACOs work to transition successfully to eCQMs and digital quality measurement. This is an excellent opportunity for ACO and DCE executives to pose questions and receive answers directly from CMS leaders and to hear the newest developments straight from the agency.
