Summary:
Connecticut operated an ACO-like program under a State Innovation Model (SIM) grant to achieve service delivery reform in the state for Medicaid and all payers, with the goal of using the ACO model to consolidate provider purchasing power. The grant program ran from February 2015-January 2020. Medicaid reforms were based on a primary care medical home program. The SIM initiatives included value-based payment efforts, care delivery innovations, consumer engagement, and health information technology improvements. Value-based payment reform initiatives under the model utilized advances care coordination and upside-only shared savings arrangements to encourage practices to embed additional features of care coordination.
